Job Description
We are seeking experienced Care Managers to join our team in Ontario. As a Care Manager with Health Med Village, you will coordinate patient care and services to ensure comprehensive and efficient care delivery.

Key Responsibilities
– Develop and implement individualized care plans for patients.
– Coordinate care and services among healthcare providers.
– Monitor patient progress and adjust care plans as necessary.
– Advocate for patient needs and ensure quality care.

Qualifications
– Certification or licensure as a Care Manager or equivalent.
– Previous experience in care management or a related field.
– Strong organizational and communication skills.
